What it is not
This PDF is not the blank published by your water supplier or local program, and it does not add a pass or fail score. It is a typed record for your own backflow test reports, not a substitute for the form used by a supplier. For report wording and layout context, see RPZ test report.
A report in Backflow Record is a record you fill in. It is not a substitute for a licensed tester or for the form your water supplier requires. It does not report an inspection result, and it does not mean a plumbing rule was met.
